About the Organization

The Elizabeth Glaser Pediatric AIDS Foundation (EGPAF) is a global leader in the fight to end pediatric HIV and AIDS. Since 2003, EGPAF has worked in close partnership with the Ministry of Health (MoH) and local authorities in Tanzania to deliver high-quality, comprehensive HIV, TB, GBV, and family planning services across supported regions.

Role Overview

The Senior Technical Advisor – Continuum of Treatment provides strategic technical leadership to strengthen client linkage, retention, treatment adherence, and continuity of care across the HIV and TB service delivery cascades in EGPAF-supported regions, health facilities, and community networks in Tanzania.

The Advisor leads the design and execution of client-centered strategies to address interruption in treatment (IIT), track and return clients lost to follow-up (LTFU), optimize community-based retention structures, and scale up Differentiated Service Delivery (DSD) models across primary and tertiary healthcare settings.

Key Responsibilities

1. Technical Leadership & Program Strategy

• Provide strategic technical direction for client-centered continuity of treatment strategies in alignment with national MoH/NASHCoP guidelines and project goals.
• Lead the formulation and implementation of comprehensive retention, adherence, and re-engagement frameworks.
• Champion the expansion and refinement of Differentiated Service Delivery (DSD) models, including Decentralized Drug Distribution (DDD), Multi-Month Dispensing (MMD), and community refill groups.
• Advise project leadership and government counterparts on evidence-based strategies to eliminate barriers to long-term treatment adherence.

2. Treatment Cascade & Interruption Prevention

• Lead routine analysis of client retention data, treatment interruptions (IIT), and re-engagement metrics across supported regions, councils, and facilities.
• Design, deploy, and monitor proactive early warning systems to identify clients at risk of defaulting or treatment interruption.
• Optimize health facility and community tracking/tracing mechanisms, including phone reminders, physical tracking, and peer navigator networks.
• Drive targeted performance improvement plans for underperforming sites, focusing on high-volume CTC facilities and vulnerable or priority populations.

3. Capacity Building & Mentorship

• Provide technical assistance, clinical coaching, supportive supervision, and mentorship to RHMTs, CHMTs, facility health workers, and community service providers.
• Develop, adapt, and deploy standardized SOPs, job aids, adherence counseling modules, and tracking tools.
• Build capacity within regional, council, and community structures to independently manage client retention and continuity of care programs.

4. Data Quality, Coordination & Learning

• Promote routine data triangulation between facility records, digital tracking registers, and community outreach data to ensure accurate continuity metrics.
• Represent EGPAF in national Retention and Adherence Technical Working Groups (TWGs), national task forces, and partner forums.
• Collaborate closely with clinical, M&E, psychosocial, and community teams to harmonize facility-community linkages.
• Document lessons learned, successful re-engagement innovations, and client-centered models to support policy advocacy and national scale-up.

Qualifications & Requirements

Required

• Education: Bachelor’s degree in Medicine (MD), Nursing, Public Health, Social Sciences, or a related clinical or healthcare discipline.
• Experience: Minimum of 8 years of progressive experience managing and leading HIV/TB client retention, adherence, and continuity of treatment programs in Tanzania.
• Technical Expertise: Advanced expertise in treatment cascade analysis, interruption in treatment (IIT) prevention, client tracking systems, Differentiated Service Delivery (DSD/DDD/MMD) models, and facility-community linkage structures.
• Stakeholder Engagement: Proven history of effective collaboration with government health authorities (MoH/NASHCoP, RHMTs, CHMTs), community-based organizations, and health facilities.

Preferred

• Advanced Degree: Master’s degree or higher in Public Health (MPH), Medical Sociology, Epidemiology, or Global Health.
• Donor Experience: Direct experience supporting PEPFAR or other major USG-funded HIV/AIDS programs in Tanzania.
