JOB DETAILS:

Duties and Responsibilities:
• Develop work plans for the day to day implementation in assigned district to achieve linkage to and retention in care for beneficiaries who are diagnosed with HIV from community-based testing efforts and ensure the activities are carried out as planned.
• Work closely with clinical implementing partners and health facility in charges to coordinate activities, share data and work plans to ensure continuum of care for beneficiaries reached or followed up in the community.
• Work closely with Monitoring and Evaluating Officer to ensure data on linkage and retention collected by the program are accurately captured and support CTC 2 Data extraction, to enable follow up and reporting on linkage and retention.
• Collects and analyses all data provided by CBHSP/PEER EDUCATORS volunteers from data sources per month.
• Improve feedback on referrals from health facilities and overall strengthen the continuum of care between community and facility intervention.
• Attending and presenting the organization in seminars or meetings and workshops involving Volunteers.
• Perform other duties assigned consistent with the placement of your position in the organization.

Minimum Qualifications:
• Bachelor’s degree in Social Sciences, information and technology, computer science.
• At least 1 year working experience in HIV/AIDS intervention program, with field work practice.
• Experience of working in rural and remote areas.
• Computer literate (Excel, Word, Power point and Internet) is compulsory.
• Having skills/knowledge of counselling is compulsory.
• Experience with (CTC 2 database systems) at Health facilities is a compulsory
• Self- motivated, team player, report writing and presentation skills, Good communication skills; fluent in Swahili and English both verbal and written
• A person with high confidentiality skills.
• Ability to work under pressure and/or tight deadlines and less supervision on achieving program result.

Position overview
Under supervision of HTS Focal person, the HTS Counsellors will play a role of Conducting HIV Testing and counselling across the range of community based settings include MJ, moonlight, Index and HIVST Testing while observing Quality and Ethical Index testing services and monitoring of the Intimate Partner Violence IPV. The HTS Counsellors will closely work with Peers educators and CBHS Providers in identified hotspots of Songwe District of Songwe Region. Will make sure all HIV Positive clients are linked for ART services to the health facilities.

Roles and responsibilities

• Participating in identification of hotspots for key and vulnerable populations
• Working closely with Peer educators and CBHS providers to sensitize the key and vulnerable groups to test for HIV to the community
• Conducting outreach HIV testing services to target groups i.e. KP, mobile testing, MJ, moonlight, HIVST and index testing services in all identified hotspots
• Documenting all HIV testing services and timely updating all registers related to HIV testing services
• Preparing weekly and monthly reports for all HIV Testing services conducted by each HTS
• ·Working closely with CBHS to identify GBV cases at the community and link at the facilities and post-follow-up.
• Identify and link clients for PrEP services and retention of clients on treatment.
• To perform any other related duties as may be assigned by the Supervisor.

Desired Qualifications
• Diploma in Nursing or clinical Medicine from health recognized college / University with relevant Authority registration. Advanced Diploma is an added advantage.
• At least two (2) years of experience working in HIV Testing and counselling services.
• Experience in writing reports related to HIV and or HIV Testing services
• Experience in basic computer applications such as MS Word, Excel, Power point and internet
• Ability to work under pressure and stringent deadlines
